The Challenge of Chronic Diseases – can we do better?
Workshop 26 May 2003

More than 1.5 million Danes suffer from one or more chronic diseases. This requires prolonged and complex efforts by hospitals and general practitioners, by the home care sector, and quite frequently also by the social services.

A workshop with Danish and international participation took place in 2003. In this forum the possibilities to improve the efforts were evaluated. It was discussed whether an improvement could be obtained by changing the way of cooperating, and by carrying out a more efficient information sharing and continuity throughout the entire course of the disease.

The workshop revealed problem areas and presented solution models applicable to the establishment of a strategy for prevention, rehabilitation and treatment of those diseases which the Danish Government classifies as national diseases in its health plan.
